Hospital discharge is good news, but it can also create a new worry: who will help your parent with transfers, medication, meals, toileting and rehabilitation after returning home?
Start with the hospital discharge plan
Before making a decision, ask the hospital or rehabilitation team what assistance your parent needs for mobility, swallowing, medication, wound care, toileting and follow-up appointments. Stroke recovery is different for every person, so the safest care arrangement depends on medical stability, functional ability and the support available at home.
When home care may be suitable
Home care may work when your parent is medically stable, the home can be made safe, a reliable caregiver is consistently available, and the family can follow the medication and rehabilitation plan.
- Someone is available to assist with daily activities.
- The bathroom, bedroom and walking areas can be made safer.
- Transfers can be managed without putting the senior or caregiver at risk.
- Family members can coordinate appointments, medication and rehabilitation.
When residential nursing care may be helpful
A more structured nursing environment may be worth considering when your parent cannot be left alone, has a high risk of falling, needs two people for transfers, has complex medication needs, or requires consistent day-and-night assistance that the family cannot safely provide.
- Frequent help is needed for bathing, toileting, feeding or mobility.
- The main caregiver is working, living far away or becoming exhausted.
- Rehabilitation and daily routines need closer coordination.
- The home environment is not yet ready for safe recovery.
A safer bridge between hospital and home
A nursing-home stay does not always have to be permanent. Short-term care can give families time to prepare the home while the resident receives daily assistance, nutritious meals, observation and rehabilitation support.

父母中风出院后，应该在家护理还是入住疗养院？
顺利出院是好消息，但也会带来新的担忧：回家以后，谁来协助父母移动、服药、吃饭、如厕和复健？
先了解医院的出院护理计划
作出决定前，应先向医院或复健团队确认长者在移动、吞咽、药物、伤口、如厕和复诊方面需要多少协助。每位中风患者的恢复情况都不同，因此合适的护理安排必须根据身体状况、活动能力以及家中可以提供的支援来决定。
什么情况下适合在家护理？
如果父母的身体状况稳定、家中能够做好安全安排、有人可以稳定照顾，并且家属能够跟进服药和复健计划，在家护理可能是合适的选择。
- 每天都有人协助日常生活。
- 浴室、卧室和走动范围可以做好防跌安排。
- 移动和转位不会让长者或照护者受伤。
- 家人能够协调复诊、药物与复健安排。
什么情况下可以考虑疗养护理？
如果父母不能独处、跌倒风险高、移动需要两个人协助、药物安排复杂，或家人无法安全地持续提供日夜照护，可以考虑较有系统的住宿护理。
- 洗澡、如厕、进食或移动都需要频繁协助。
- 主要照护者需要工作、住得远或已经身心疲惫。
- 复健与日常作息需要更有规律地协调。
- 家中暂时还没有准备好安全的康复环境。
作为医院与回家之间的安全过渡
入住疗养院不一定代表长期居住。短期护理可以让家人有时间准备居家环境，同时让长者获得日常协助、营养膳食、情况观察和复健支援。
